Health Inventory Form
The Health Inventory Form gathers a child’s medical history, allergies, and current health status for daycare records
Allergy Action Plan
The Allergy Action Plan Form outlines steps to manage a child’s allergies, including triggers, symptoms, and emergency treatment.
Medical Authorization Form
The Medical Authorization Form gives our daycare staff permission to seek medical treatment for a child in case of an emergency.
Asthma Medication Authorization Form
The Asthma Medication Authorization Form grants permission for our daycare staff to administer prescribed asthma medication to a child as needed
Maryland Authorization Certification Form
The Maryland Authorization Certification Form allows parents to authorize us to administer medication or medical treatments to their child, in compliance with state regulations
